The Rhode Island Office of the Attorney General is asking for the public's help during the ongoing COVID-19 pandemic. If you have firsthand knowledge of illegal activity or other misconduct involving a Rhode Island-based nursing home or long-term care facility during the COVID-19 pandemic, please complete the form below.

If you have documents that demonstrate the misconduct you are reporting, you may upload them through the form. If you prefer, you can remain anonymous, but the more detailed your submission, the more helpful it will be for investigators.

Please be aware that while anonymous complaints will be thoroughly investigated to the best of our abilities, in most instances, investigators have follow-up questions that can assist in the investigation and can only be answered by the complainants. If you desire, during the process of speaking with investigators, you will be able to remain anonymous. Rhode Island General Laws are in place to protect “Whistleblowers”. Employers may not take retaliatory actions or discriminate against employees who file complaints against them.
