Registration Forms for Health Clubs and Telemarketers

Health Clubs

Did you know? Under Rhode Island General Law 5-50, health clubs must register with the Office of Attorney General. There is a non-refundable registration fee of $100. Registration is valid for one year from the date of approval and the fee must be submitted with the completed registration form provided below.

Registration may be annually renewed by filing an updated registration form and paying a renewal fee of $50.

Telemarketers

Rhode Island General Law 5-61, “The Telephone Sales Solicitation Act,” requires all telephone sales solicitation operations, or telemarketers, to register with the Office of Attorney General not less than 10 days prior to doing business in Rhode Island. There is a non-refundable registration fee of $100. Registration is valid for one year from the date of approval and the fee must be submitted with the completed registration form provided below.
