The Rhode Island Office of the Attorney General is asking for the public's help during the ongoing COVID-19 pandemic. If you have firsthand knowledge of illegal activity or other misconduct involving a Rhode Island-based nursing home or long-term care facility during the COVID-19 pandemic, please complete the form below. If you have documents that demonstrate the misconduct you are reporting, you may upload them through the form.
